GALL BLADDER SURGERIES
Gall Bladder surgery mostly involves removal gall bladder (Cholecystectomy). It is mostly performed to treat the gallstones that form in the gall bladder and cause symptoms or complications. It is also done to treat gall bladder cancers and also as part of certain major liver surgeries (Hepatectomy) even if it is not diseased. Cholecystectomy
A cholecystectomy is mostly performed by key hole technique (laparoscopically) by inserting a tiny video camera and special surgical tools through three to four small incisions inside the abdomen. Most patients are able to go home the same day of surgery, sometimes a single day stay in the hospital is needed and it takes about a week to fully recover.
Radical Cholecystectomy
A Radical cholecystectomy is performed in cases of proven gall bladder cancers. In this surgery along with gall bladder a part of liver adjoining it and the complete lymphatic channels around the liver hilum are removed. This may very occasionally involve complete resection and reconstruction of biliary tree also.
COMPLICATIONS
Cholecystectomy carries a small risk of complications including:
1. Bile leak (1 in 100)
2. Bleeding
3. Infection
4. Injury to nearby structures, such as the bile duct (1 in 500), liver and small intestine
5. Other general complications of surgery
OTHER BILE DUCT SURGERIES
Surgeries over the bile duct involves either clearing the bile duct of stones( CBD Exploration), or creation of a bile duct bypass (Hepaticojejunostomy / Hepaticoduodenostomy) with or without bile duct removal (CBD excision).
CBD exploration
CBD exploration is performed to remove stones from inside the bile duct. It almost always involves removal of gall bladder also.
Bile duct excision and reconstruction
Bile duct excision is required for cysts (Choledochal cysts) or cancers of the bile duct. If bile duct is being excised for bile duct cancers it may also involve removal or part of liver (if it is located in upper part of bile duct) or pancreas and duodenum (if located in lower part of bile duct).
Bile duct reconstruction/ Bypass
Bile duct reconstruction or bypass is required to correct injuries of the bile duct (traumatic or iatrogenic), bile duct strictures or narrowing, and to relieve biliary obstruction in cases of advanced pancreatic, stomach or gall bladder cancers when the cancer cannot be surgically cured.
